Privacy Notice

Effective: August 11, 2026. This notice describes the founding RemakeRx website and 90-Day Audit workflow. It should be read together with the customer-specific scope/order confirmation.

Website inquiries

When you request information or a founding audit, RemakeRx may collect the business contact details you submit, such as your name, dental-laboratory name, work email, and approximate monthly case volume. We use these details to respond to the request, qualify the audit, and operate the customer relationship.

Audit data: de-identified and minimized

The founding audit is designed to receive only the minimum de-identified operational data needed for remake-pattern analysis. RemakeRx does not ask for patient names, dates of birth, addresses, patient phone/email, insurance/member identifiers, medical-record identifiers, clinical images, or patient-identifying free text for this pilot.

Customers are responsible for determining and applying the de-identification method appropriate to their legal obligations before transfer. If the data is not clearly de-identified or contains unnecessary identifiers, RemakeRx will pause intake and request a minimized export rather than analyze it.

Secure transfer and retention

Qualified audit customers receive a customer-specific secure upload link. Founding-audit files are intended to be stored outside the public website directory with restricted access. RemakeRx uses a limited retention workflow and may delete working files after the agreed audit/measurement period unless a different written arrangement applies.

How we use audit data

Audit data is used to validate data quality, calculate agreed operational metrics, identify descriptive concentration patterns, prepare the RemakeRx Diagnostic, and support the agreed corrective-action and measurement workflow. RemakeRx does not use the founding audit to provide clinical diagnosis or treatment recommendations.

Sharing

RemakeRx does not sell customer audit data. Service providers may process limited information where needed to operate hosting, email, payments, or other business infrastructure, subject to the configuration and agreements actually in place.

Operational note: Before expanding beyond the de-identified founding workflow, RemakeRx should obtain jurisdiction-specific legal/privacy review for the actual data, hosting, customer agreements, and regulated-role assumptions involved.
