Founding Audit Terms

Effective: August 11, 2026. These terms summarize the RemakeRx 90-Day Audit founding workflow. Customer-specific scope, price, timing, payment terms, and any additional commitments are confirmed in writing before work begins.

Service scope

The founding audit is an operational quality-analysis service using an agreed de-identified case/remake dataset. The standard deliverables are a data-readiness review, baseline remake/first-pass metrics, concentration and interaction analysis, evidence-linked investigation priorities, a corrective-action register, and a 30-day measurement plan.

Start condition

Audit work begins after the agreed fee is received and the submitted dataset passes the RemakeRx data-readiness check. If required fields are missing or the data is not suitable for the founding workflow, RemakeRx will identify the issue before substantive analysis.

Customer responsibilities

The customer is responsible for having authority to provide the data and for ensuring the founding-audit export is appropriately de-identified and minimized before transfer. Do not send patient-identifiable data, unnecessary clinical records, or clinical images through the founding upload workflow.

Analytical limitations

RemakeRx distinguishes descriptive evidence, operational hypotheses, and recommended tests. Concentration, correlation, and over-indexing do not by themselves prove root cause. The customer remains responsible for operational decisions and for validating any proposed intervention in its own workflow.

No clinical service

RemakeRx provides operational quality intelligence and does not provide diagnosis, treatment planning, or clinical dental advice.

No guaranteed outcome

The audit guarantees the agreed analysis and deliverables, not a specific remake-rate reduction, financial saving, or other business result. The 30-day measurement step is intended to determine whether selected actions appear effective, ineffective, or inconclusive.

Confidentiality and data handling

RemakeRx will handle customer data according to the agreed founding workflow and privacy notice. Any requirement to process patient-identifiable information, regulated clinical records, or materially different data requires a separate written and technical arrangement before transfer.
